Form Expiration

Important Information About Your Forms & Portal

Some forms, such as consent forms, may have an expiration date. While patients would see that they have a new form to fill out when you reset it, the system does not automatically alert our office when a form expires. Our administrative team sets manual reminders to periodically update these forms for you.
